New Patient Registration

You can register either online using our secure online forms or in person.

 

Accepting and Removing Patients from Our List

We can only accept new patients who live in our practice area, if you register with us and are then found to be out of our catchment area, we will request that you register with a practice closer to you.

If you would like to register in person, please come to the surgery on Monday to Friday, between 9am and 6.30pm.

We may ask to see proof of ID and/or address in order to ensure we register you with the correct details - however we will always register you even if you do not have these documents. You will need to provide photo ID, proof of address and an NHS number if you want to register online

All newly registering patients over the age of 40 will be booked to see the healthcare assistant for a new patient health check.  This will make sure our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up to date.

If you are registering children under 5 years old, please let us know what childhood immunisations they have received.

If you move out of our area please let reception know as we may be able to keep you registered, depending on your new address, otherwise we will ask you to re-register with a more local practice.

 

NHS Health Checks

Following your registration, we would like to book you in for a health check with our nurse or health care assistant, please ask reception about this.

 

Named Accountable GP

From 1 April 2015, practices are required under the GMS contract, to allocate a named accountable GP to all patients, including children. All patients registered before 1 April 2015 will be allocated a GP before the end of June 2015.

The named GP will have overall responsibility for your care. Please ask reception for the name of your accountable GP.
