You can register either online or in person.
New Patient Registration
Accepting and Removing Patients from Our List
We can only accept new patients who live in our practice area, if you register with us and are then found to be out of our catchment area, we will request that you register with a practice closer to you.
If you would like to register in person, please come to the surgery on Monday to Friday, between 9am and 6.30pm.
The NHS does not require you to provide photo ID, proof of address or an NHS number to register with a GP surgery however, it helps us to accurately identify you during the registration process and keep your records up to date. You will need to provide photo ID, proof of address and an NHS number if you want to register online.
All newly registering patients over the age of 40 will be booked to see the healthcare assistant for a new patient health check. This will make sure our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up to date.
If you are registering children under 5 years old, please let us know what childhood immunisations they have received.
If you move out of our area please let reception know as we may be able to keep you registered, depending on your new address, otherwise we will ask you to re-register with a more local practice.
NHS Health Checks
Following your registration, we would like to book you in for a health check with our nurse or health care assistant, please ask reception about this.
